| I. Background | |
| ERBB 家族(又稱 HER家族)是一類在細(xì)胞生長、分化與存活中至關(guān)重要的受體酪氨酸激酶,其異常激活是驅(qū)動多種癌癥(如肺癌、乳腺癌、胃癌)的關(guān)鍵因素。該家族包含 四名成員:EGFR/HER1、HER2、HER3 和 HER4。它們各自的特點(diǎn)為:EGFR 擁有多種配 體;HER2 無已知直接配體但信號能力最強(qiáng);HER3 激酶活性缺失,必須與其他成員協(xié)作; HER4 結(jié)構(gòu)最為復(fù)雜。其中,HER2 是最核心的信號放大器,而 HER3 是關(guān)鍵的信號伙伴, 兩者的異源二聚體是驅(qū)動腫瘤生長和耐藥的主要信號單位。 ERBB 家族通過配體誘導(dǎo)的二聚化機(jī)制啟動其核心信號通路。當(dāng)生長因子(配體)結(jié)合后,受體形成同源或異源二聚體(尤其是以 HER2 為中心的異源二聚體),引發(fā)胞內(nèi)酪氨酸激酶區(qū)的相互磷酸化。這些磷酸化位點(diǎn)隨后募集并激活下游兩條主要信號通路: RAS/MAPK 通路(調(diào)控基因表達(dá)和細(xì)胞增殖)與 PI3K/AKT 通路(調(diào)控細(xì)胞存活和代謝)。這條從膜受體到細(xì)胞核的強(qiáng)大信號傳導(dǎo)級聯(lián),在正常情況下精密調(diào)控生理過程,一 旦因受體過表達(dá)或突變而失控,便會持續(xù)驅(qū)動腫瘤的生長、侵襲和生存,這也正是針對 EGFR 和 HER2 等靶點(diǎn)的藥物(如單克隆抗體、小分子抑制劑)能夠有效治療癌癥的根本原理。 |
